A bidder or proponent desiring to make an application to the chief of public entity pursuant to Section 47 of the Act for the review of procurement action or decision shall specify the following matters in such an application and sign it:
(a) Name, address, telephone, fax number and e-mail of the applicant,
(b) If the applicant is a corporate body, power of attorney given by that body to make such an application,
(c) Bid number,
(d) Factual and legal grounds for making application,
(e) Copy of the relevant document.
